Intake Assessor (LSW, LISW, LPC, LPCC)

Job Description

We are seeking a dedicated Intake Assessor to join our team at The Crossroads Center. In this role, you will be responsible for conducting psychosocial assessments, determining appropriate levels of care, and making referrals to specialized programs tailored to meet the individual needs of our clients.

Key Responsibilities

• Conduct psychosocial assessments for individuals seeking services at The Crossroads Center.
• Utilize diagnostic tools to make informed decisions on the most suitable levels of care.
• Collaborate with clinical staff to address any challenges during the intake process.
• Maintain accurate documentation in line with agency standards.
• Provide exceptional customer service to all stakeholders.

Experience & Qualifications

• Bachelor's degree in social work, counseling, or related field; Master's degree preferred.
• Minimum of 2 years of intake and assessment experience with substance use and mental health disorders.
• Required Certifications: LSW, LISW, LPC, LPCC in the State of Ohio.
