Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health in Baltimore, MD. As part of our allied health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and families. Your responsibilities include addressing various challenges such as adjustment to institutional living, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and address financial concerns.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- State licensure such as LCSW-C and LGSW.
- Experience in a healthcare setting is a plus.
