Job Description

Responsibilities
- Assess and address common psychosocial needs of patients and families.
- Provide counseling on various challenges including adjustment to medical conditions, financial concerns, and end-of-life issues.
- Contribute to discharge planning and support patients through transitions.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification required.
