SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and their families. This includes addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to various life challenges.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments to address common problems.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's in Social Work (BSW) required.
• Master's degree in Social Work preferred.
• LCSW-C certification preferred.
• Experience as an LGSW.
