Responsibilities

• Provide skilled nursing care and treatment to geriatric patients in the community.
• Implement goal-oriented and patient-centered care plans.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.

Job Details

• Job Types: Per diem, Part-time, Full-time
• Pay: $22.00 - $28.00 per hour
• Expected hours: 20 – 40 per week
• Medical Specialties: Geriatrics, Home Health, Pediatrics, Wound Care
• Standard Shift: Day shift, Night shift
• Supplemental Pay Types: Differential pay
• Supplemental Schedule: Holidays, Overtime
• Weekly Schedule: 5x8, Monday to Friday, Rotating weekends
