We are seeking a dedicated Family Nurse Practitioner or Physician Assistant to join our innovative team at One Medical. As a provider, you will have the opportunity to manage a diverse patient panel, including acute, chronic, and preventive care visits. You will collaborate with a supportive team to deliver exceptional care through in-office, testing center, and tele-health visits.

Responsibilities

• Manage a patient panel with various healthcare needs.
• Deliver in-office and tele-health visits for a comprehensive patient approach.
• Collaborate with in-office and virtual clinical teams to enhance patient care.
• Utilize your clinical training for in-office procedures and continuous learning.

Requirements

• Completed accredited FNP or PA program with national certification.
• Minimum 2 years of experience in outpatient primary care settings.
• State licensed in New York by the start date.
• Passion for human-centered primary care and effective communication skills.
