Job Description
We are seeking a dedicated Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse in our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community, assisting with care plans and discharge planning, supporting New Castle County, DE. Our organization offers a state-of-the-art Electronic Health Record system with comprehensive OASIS-E training to help you excel in your career.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.

Schedule
