Job Description
We are looking for an experienced Home Health Licensed Practical Nurse (LPN) to join the ChristianaCare team. As a member of our HomeHealth division, you will provide skilled nursing care and treatment to geriatric patients in the community. In this role, you will have the opportunity to implement patient-centered care plans and contribute to discharge planning.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
Job Types

Pay
$22.00 - $28.00 per hour

20 – 40 per week
