Are you an experienced Home Care LPN looking to join a dynamic team in a supportive environment? ChristianaCare is seeking Full Time, Part Time, and Per Diem LPNs to provide skilled nursing care to geriatric patients in the community. Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement patient-centered care plans consistent with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma (HSD) or GED required.
• Must possess a current DE LPN license or compact state license.
