Job Description
We are looking for experienced Home Care LPNs to join our innovative team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.

- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
