Job Description
We are currently seeking experienced Home Health Licensed Practical Nurses (LPNs) to join our team at ChristianaCare. As part of our dedicated Home Health division, you will have the opportunity to provide skilled nursing care and treatment to geriatric patients in the community. Our innovative Electronic Health Record system and comprehensive training ensure you have the tools for success in your Home Health career.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
