Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As part of our dedicated team, you will provide skilled nursing care and treatment to geriatric patients in the community. Our Electronic Health Record system and comprehensive training ensure you have the tools to excel in your role.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
