Licensed Practical Nurse MDS Coordinator

Job Description

Genesis Health is currently seeking a Licensed Practical Nurse MDS Coordinator to join our dedicated team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• LPN license in good standing.
• Prior experience in hospital or long-term care facilities.
• Proficiency in general, rehabilitative, and restorative nursing practices.
• Knowledge of laws, regulations, and guidelines in long-term care.
• Computer skills with proficiency in Windows applications.
