Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our Home Care program, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to the discharge planning process. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
- Available for full-time, part-time, or per diem positions.
