Job Description

Responsibilities
- Provide skilled nursing care to geriatric patients in the community.
- Implement patient-centered care plans within available resources.
- Participate in developing discharge plans.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
Job Details
- Job Types: Per diem, Part-time, Full-time.
- Pay: $22.00 - $28.00 per hour.
- Expected Hours: 20 – 40 per week.
