Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our Home Health team at ChristianaCare. As an LPN in our organization, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Must possess a current DE LPN license or compact state license.
