Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our ChristianaCare HomeHealth team. You will have the opportunity to provide skilled nursing care to patients in the community, supporting New Castle County, DE. As part of our team, you will work collaboratively to implement care plans and ensure the well-being of our patients.
Responsibilities
- Deliver skilled nursing care and treatments to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
