Job Description

We are looking for a detail-oriented Minimal Data Set Coordinator - LPN to join Genesis Health, one of the leading healthcare services providers. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) to ensure proper patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Participate in developing individualized resident Care Plans.
• Train new team members on clinical content and MDS completion.

Requirements

• Active LPN license.
• Prior experience in hospital or long-term care settings.
• Proficiency in general nursing practices and MDS completion.
• Computer skills with knowledge of Windows applications.
