Nurse-LPN MDS Coordinator

Job Description

We are looking for a dedicated Nurse-LPN MDS Coordinator to join Genesis Healthcare, a leading provider of healthcare services. As the LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights

• Direct the interdisciplinary RAI process and ensure accurate completion, encoding, and transmission of MDS.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Provide training on clinical content and electronic completion of MDS items to new team members.

Requirements

• LPN license in good standing.
• Prior experience in hospital or long-term healthcare settings.
• Proficiency in general nursing practices and medical procedures.
• Knowledge of long-term care regulations and guidelines.
• Computer proficiency, including Windows applications.
