Physical Therapist Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct thorough home visit Physical Therapy assessments on referred patients.
• Proficient in EMR documentation, experience with Hello Note is advantageous.
• CPR Certification.
