Physical Therapist Responsibilities

• Conduct in-home Physical Therapy assessments and provide direct patient care.
• Document patient progress accurately using EMR systems.
• Collaborate with patients to assess medical conditions and rehabilitation potential.

Physical Therapist Requirements

• Graduated from an accredited Physical Therapy program.
• Hold a valid, unencumbered PT license in New York.
• Proficient in EMR documentation.
• Current CPR Certification.
