Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments to evaluate medical conditions, functional abilities, limitations, and rehabilitation potential.
• Proficient in EMR documentation (experience with Hello Note is beneficial).
• CPR Certification.
