Physical Therapist Responsibilities:

• Assess patients' medical conditions, functional capabilities, and potential for rehabilitation through home visits.
• Provide direct patient care and create personalized treatment plans.
• Utilize EMR documentation for accurate record-keeping.

Physical Therapist Requirements:

• Graduation from an accredited Physical Therapy (PT) program.
• Valid Physical Therapy (PT) license in New York.
• Proficiency in EMR documentation, preferably with experience in Hello Note.
• CPR Certification.
