Physical Therapist Requirements:

• Graduate of an accredited Physical Therapy program.
• Valid Physical Therapy license in New York.
• Conduct comprehensive home visit assessments and deliver direct patient care.
• Proficiency in EMR documentation (experience with Hello Note a plus).
• CPR Certification.
