• Conduct home visit assessments of patients and provide direct care to evaluate medical conditions and rehabilitation potential.
• Proficiently document patient information using EMR system.
• Flexible scheduling options available, including full-time, part-time, and per diem.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license for New York.
• CPR Certification.
• Experience with EMR documentation preferred.
