Job Description
We are looking for a compassionate and dedicated Registered Nurse (RN) to join our team at Atrium Health. This position involves providing exceptional care to adult and geriatric patients in the comfort of their own homes. As part of the home care team, you will play a critical role in managing post-acute services, coordinating care with multidisciplinary teams, and ensuring high-quality outcomes for our patients.
Job Responsibilities
- Serve as a case manager for an assigned group of patients, overseeing post-acute services in the home setting.
- Provide direct patient care and supervise LPNs and HHAs according to established care plans.
- Coordinate care with the multidisciplinary team and act as the patient's liaison with other healthcare providers.
- Ensure timely and qualified patient care, documentation, and high-quality outcomes.
Qualifications
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license and BLS certification required.
- Experience in home health care or similar setting is a plus.
- Strong critical thinking, communication, and leadership skills.
