Responsibilities

• Conduct home visit assessments of referred patients to evaluate medical conditions and rehabilitation potential.
• Provide direct patient care to assess functional capabilities and limitations.
• Document patient interactions using EMR systems.

Requirements

• Graduation from an accredited Physical Therapy program.
• Unencumbered Physical Therapy license in New York.
• CPR Certification.
