Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and rehabilitation potential.
• Provide direct patient care and create personalized therapy plans.

Requirements

• Graduated from an accredited Physical Therapy program.
• Hold a valid Physical Therapy license for New York.
• Proficient in CPR and EMR documentation.
