Paradigm Rehab is looking for a dedicated Rehabilitation Therapist to join our team. Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and potential for rehabilitation.
• Provide direct patient care focused on assessing functional capabilities and limitations.
• Proficiency in EMR documentation (experience in Hello Note preferred).
• Maintain an unencumbered Physical Therapy (PT) license for New York.

Requirements

• Graduation from an accredited Physical Therapy (PT) program.
• Current CPR Certification.
• Willingness to relocate to Brooklyn.
